HMCTS says Form N265 is used where the court has ordered standard disclosure and says not to use it for a claim allocated to the Small Claims Track.
Practice Direction 31A describes Form N265 as the normal disclosure-list form. Applicability and substantive decisions remain with the customer or adviser.

Practice Direction 31A describes date order, consecutive numbering and concise descriptions as normal list practices, with grouping possible for large similar categories.
Court orders, current rules and the official form remain authoritative.

Scope may include indexing, date organisation, duplicate or version notes, working descriptions and reviewable N265 list material.
This does not mean every civil matter needs N265 or that any substantive legal decision is included.
